Restorative Justice Map Submission Form

Please note: 

• At least one category (i.e., Coalitions, Direct Service Providers, K-12 Schools, Colleges and Universities, Consultants/Independent Contractors) needs to be selected. If more than one category applies, please select all that apply. For a description of categories, please visit the About page.  
• For an entry to appear on the map (i.e., a pinpoint), a physical (street) address needs to be listed as the "Mailing Address" as Google Maps does not accept P.O. Box addresses. If a physical (street) address is not provided, your entry will not show up on the map. The entry will however still show up in the directory.  
• Only submit a physical (street) address if you are comfortable with your location pinpoint showing up on the map. This may be especially relevant for those (e.g., independent contractors) who utilize a home address for their business. If you provide a physical (street) address and want it removed at any point, please contact info@nacrj.org.
